Registration Information
All:
Just a reminder that we will have registration this Tues. 1/24/12 from 7:00 to 9:00 p.m. at Taylorview. Then Monday 1/30/12 from 7:00 to 9:00 p.m. we will have registration and equipment rental. If you have old equipment you would like to sell, you can bring it, put your own price tags on it and sell it yourselves. Please pass the word out on registration times and dates to anyone who hasn't played in the past. Additional registration times will be announced by email and the calender if held.
If you would be available to help, please respond to me by e-mail letting me know which date works for you. We will be asking those who do not volunteer to help with the many tasks of the league to donate $25 instead of their time. The Board cannot run this league without your help, so please respond!
Registration Fees:
- All players are required to register with US Lacrosse ($25 Junior High, $35 High School) - No Exceptions - we will need the ID number in order to complete registration. Register on-line at www.uslacrosse.org. Registration supplies some insurance coverage and the Lacrosse Magazine.
- Registration for the Idaho Falls Lacrosse Club is $60.00, which is due before the official start of the 2012 season.
- Boys' varsity and junior varsity are required to register with Utah Lacrosse Association -$112 if paid before 2/15 - No Exceptions. The price goes up significantly the later your register - $140 till 2/28, then $190). Scholarships are available. Register on-line at www.utahlax.org. Registration pays for the referees at all games and supplies us with games to play. You can also find the schedule at this web site - we are Division B - North.
Equipment rental, and deposit fees are separate from the registration fee.
Equipment-Boys
Equipment (helmet, shoulder and arm pads) is available for rental, with first year players receiving preference. Rental cost is $50, with $250 deposit (the deposit is paid by a separate check that we will not deposit, but will hold until equipment is returned). We will also be renting equipment on a piece-by-piece basis, as available. Equipment rental will take place at Taylorview on 1/24 and 1/30 from 7 to 9 pm. Returning players are encouraged to purchase their own equipment.
All players must have their own sticks, colored mouth guards and cleats.
- Boys need athletic supporter/cup.
- Girls need goggles

PARTICIPATION AGREEMENT:
I hereby register my child listed above for the Idaho Falls Lacrosse Club, Inc. 2010 Lacrosse season. Volunteer coaches will direct him in all lacrosse program activities. By signing below, I acknowledge that my child has no medical or emotional problems that may affect his ability to safely participate in the Lacrosse program. I also authorize IFLA volunteers to attend to any health problems or injuries my child may incur while participating in the Lacrosse program. I also give my consent for all medical care prescribed by a duly licensed Doctor of Medicine for the above minor as his legal guardian. This care may be given under any condition deemed necessary to preserve the life, limb, or well being of the above minor. I recognize that limited insurance is provided to my child after registering with US Lacrosse and that any additional coverage is my responsibility. I recognize that my child is at risk of receiving a severe or crippling injury, even death, while playing lacrosse. I agree that the Idaho Falls Lacrosse Club Inc. coaches, officials, players and volunteers will not be liable for any injury to my child while participating in the program.
VOLUNTEERS NEEDED
In order to successfully run our program, Volunteers are required. We are always looking for parents to help. Your commitment is often minimal, yet its impact makes a huge difference and is appreciated by all. Please initial next to a duty you would like to volunteer for below. If you are unable to volunteer, IFLA will accept a donation of $25.00 as an alternative. Thank you again for your support.
